Major Change Form

This form is for currently enrolled UH students ONLY.

If you are a newly admitted (freshman, transfer or readmit) student for Summer/Fall 2025, please contact the Office of Admissions at 713-743-1010 for a major change request. Your request will be voided if you are a Summer/Fall 2025 applicant.

Requests for a second degree must be made in writing on a General Petition form and submitted via email to Mr. Jemal Moore, jmoore [at] central.uh.edu (jmoore[at]central[dot]uh[dot]edu).

If you would like to void your major change submission, please email engrugpr [at] central.uh.edu (engrugpr[at]central[dot]uh[dot]edu).

Deadline for Fall 2025: August 1, 2025 at 11:59PM

You must be logged into your AccessUH to view and submit the form.

Students may expect a decision to be rendered via email prior to the beginning of the Fall 2025 semester.
