Courtesy Appointments (499)

Courtesy Appointments are appointments to the university that need to have all of the following documentation and need to be submitted to the Dean’s office as soon as possible.

  • Approval of Hiring
  • Offer Letter
  • CV
  • Faculty Employment Acceptance form – with section I, II, III signed
  • Personal Data Sheet
  • CHRI Clearance http://www.uh.edu/human-resources/manager-toolbox/CHRI_request/
  • Copy of Original Transcripts – Original Transcripts need to be turned in to Dean’s office prior to submitting ePAR. Online transcripts with terminal degree need to be email directly to facultyaffairs [at] uh.edu. The department is responsible for walking over the transcripts to dean’s office.
  • Justification if needed.

All non-tenure hard copy packages need to be completed before submitting to the Dean’s office. Any package that is incomplete will be sent back to department.
